Return Policy Consent Form

Purpose

This form outlines the return, refund, and cancellation policy for all medical and wellness programs offered by MediDuce Wellness, including prescription and compounded medications such as Semaglutide, Tirzepatide, and other peptide-based treatments.

By ticking the box you acknowledge that you have reviewed and agree to these terms prior to starting your program.

General Policy

Because MediDuce Wellness provides personalized medical services and prescription-only medications, all sales are considered final once the prescription or medication order has been processed.

Refunds & Returns

Consultation Fees:
The $50 consultation fee is non-refundable, regardless of treatment eligibility or program continuation.

Medication Orders:
Once your medication has been prescribed, compounded, or shipped, it cannot be returned or refunded under federal law, even if unused.

Program Packages:

Monthly, Quarterly, and Semi-Annual Plans:
Once treatment begins and the first medication order has been processed, these packages are non-refundable.

Annual Plans:
Annual prepaid programs are eligible for a 50% refund of the remaining unused months if canceled before the second shipment or refill has been dispensed.
No refunds will be issued after multiple doses or shipments have been completed.

Cancellations:
Cancellations must be requested before your medication order is processed by the pharmacy.
Once the order is placed or compounded, the sale is final.

Reshipments or Lost Packages

If a package is lost or damaged during shipment:

Notify MediDuce Wellness within 7 business days of the expected delivery date.

Replacement or reshipment requests will be reviewed case-by-case and may require pharmacy verification.

Billing Errors or Duplicate Charges

If you believe you have been charged in error, please contact [email protected] within 5 business days of the charge appearing on your account for correction.

By ticking the box, you confirm that:

You have read and fully understand MediDuce Wellness’s return and refund policy.

You understand that once medication is prescribed or shipped, the sale is final.

You acknowledge that annual programs are eligible for a 50% refund only under the conditions described above.

You consent to these terms as part of your treatment and payment agreement.
